Course Setup Form

• To request a Connect 4 Education course, please complete the Course Setup Form.
• Your course will be ready within 5 to 10 business days, and you will be notified once it's completed.
• Your course will be available on the course site at www.c4edu.org.

Please Note: You will need to complete the Course Setup Form prior to each semester you will be teaching with us.
